Employee Benefits Summary
Medical Insurance
Available the first of the month following 60 days of employment

Flex Spending – Section 125 Plan
Flexible Spending Account for payment of health care and dependent care expenses with employee pre-tax earnings.
Dental Insurance
- Available the first of the month following 60 days of employment
- Group rates with free check-ups twice per year
- Coverage for diagnostic and preventative at 100%
- Basic restorative services covered at 80%
- Major services covered at 50%
Vision Insurance
Available the first of the month following 60 days of employment
- Group rates with $10 exam fee
- Coverage for eyeglasses and contacts
Safety Glasses
FVMT will reimburse employees needing prescription safety glasses $150 once every calendar year.
Safety Shoes
FVMT will reimburse employees up to $150 once every calendar year for the purchase of safety to shoes.
Life Insurance
Available the first of the month following 60 days of employment
- FVMT provides $20,000 basic life insurance and pays 75% of premium
- Supplemental coverage for self, spouse, and children is available at group rates
401(k) Retirement Plan
Starts the first of the month following 90 days of employment
- Automatic enrollment at time of hire.
- FVMT provides matching funds with investment by employee
Paid Holidays
Employees are provided 9 paid holidays each calendar year
- Standard paid holidays are New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and day after Thanksgiving, and Christmas
- Two floating holidays are used with standard holidays on a calendar year basis
Paid Time Off/Vacation
Prorated the first year of employment based on start date and then follows company plan based on years of service. Opportunity to earn additional time off with participation in Safety Plan.
Profit Share Bonus
Annual amount based on company performance and profitability. Increased to 150% if invested in 401(k) Retirement Plan
Short Term Disability Insurance
Available the first of the month following 60 days of employment
- Provides employee 60% of wages (maximum of $600 per week) for disability outside-of-work or illness
- FVMT pays 75% of short term disability insurance plan premium
Long Term Disability Insurance
Available the first of the month following 60 days of employment
- Provides employee 60% of wages (maximum of $2500 per month) for disability outside-of-work or illness
- Voluntary insurance at group rate
