Medical Insurance

  • Three Medical plans offered; Two are high deductible, lower premiums plans
  • Team Members have the opportunity to save money on bi weekly premiums by participating in our Wellness Program

Flex Spending – Section 125 Plan

  • Flexible Spending Account for payment of healthcare and dependent care expenses with employee pre-tax earnings

Dental Insurance

  • Two different dental plans are offered
  • Group rates with free check-ups twice per year
  • Coverage for diagnostic and preventative testing at 100%
  • Basic restorative services covered at 80%
  • Major services covered at 50%

Vision Insurance

  • Group rates with $10 exam fee
  • Coverage for eyeglasses and contacts

Life Insurance

  • FVMT provides $20,000 basic life insurance free to our Team Members
  • Supplemental coverage for self, spouse, and children is available at group rates

Short Term Disability Insurance

  • Available the first of the month following 60 days of employment
  • Provides employee 60% of wages (maximum of $600 per week) for disability outside-of-work or illness
  • FVMT pays 75% of short term disability insurance plan premium

Long Term Disability Insurance

  • Available the first of the month following 60 days of employment
  • Provides employee 60% of wages (maximum of $2500 per month) for disability outside-of-work or illness
  • Voluntary insurance at group rate

401(k) Retirement Plan

  • Starts the first of the month following 90 days of employment
  • Automatic enrollment of 6% at time of hire
  • FVMT provides matching funds with investment by Team Member

Paid Holidays

  • Employees are provided nine (9) paid holidays each calendar year
  • Standard paid holidays are New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and day after Thanksgiving, and Christmas
  • Two floating holidays are used with standard holidays on a calendar year basis

Paid Time Off/Vacation

  • Prorated the first year of employment based on start date and then follows company plan based on years of service
  • Opportunity to earn additional time off with participation in FVMT’s Safety Plan

Profit Share Bonus

  • Annual amount based on company performance and profitability
  • Increased to 150% if invested in 401(k) Retirement Plan

Safety Glasses

  • FVMT will reimburse Team Members $150 once every calendar year for employees requiring prescription safety glasses

Safety Shoes

  • FVMT will reimburse Team Members up to $150 once every calendar year for the purchase of safety toe shoes
